Job Description

Position Objectives

The Community Mental Health & Housing Practitioner supports individuals who are homeless or at risk of homelessness by providing short to medium-term psychosocial support and case management.

Integrated within Mallee Family Care’s Mental Health services, the role focuses on assisting people to access and sustain long-term housing, build independent living skills, improve mental health and wellbeing, and, where appropriate, transition to the NDIS. Working from a recovery-oriented framework, the practitioner promotes self-determination, connects people with housing options, education, employment, and other relevant services, supports families and carers, and aims to reduce the likelihood of clinical (re)admission.

Desired Skills and Experience

Qualifications and Experience:

  • The person employed will be expected to have previous relevant experience in the field and will be required to hold an accredited Tertiary qualification relevant to the position at Degree Level or above in Mental Health, Psychology or Social Work. Diploma level qualification in relevant field will be considered.
  • People with a lived or living experience (carer or consumer) of public mental health services are encouraged to apply.
  • A current driver’s licence and a willingness to travel and preparedness to be accommodated overnight when required.
